With a compassion for people, commitment to service, motivation to affect change, cultural awareness and knowledge about your role and responsibilities as a NON-MEDICAL CASE MANAGER / PrEP NAVIGATOR (NMCM/PN) you will help to fulfill this mission by utilizing your knowledge, training and experience to assist HIV/AIDS diagnosed individuals in obtaining needed services. The NMCM/PN will work with individuals vulnerable to HIV by providing PrEP information, linkage to insurance navigation services if needed, linkage to PrEP care/treatment and follow –up for outcome of PrEP service, treatment and care.
Through short term case management the NMCM/PN will assist clients with referrals and linkages to medical, social, legal, financial, mental health and/or substance abuse referrals and Benefits/entitlement counseling. They will help individuals who are in need of information and guidance obtain essential supportive services that will help clients stay adherent to primary HIV care and treatment to improve health outcomes and supporting clients’ independence and self-sufficiency.
