AHF is seeking a full-time regular Disease Intervention Specialst to work in our Public Health Division.  The hours can vary and are usually Monday - Friday 12pm - 9pm.



  • Conducts on going community assessments to map out locations and hours to schedule the MTU.
  • Produces monthly calendar where the MTU will be providing services.
  • Coordinates locations and hours where testing will be scheduled.
  • Prepares all documents, forms and supplies needed during testing.
  • Processes all required documents internally and to any external agencies needed.
  • Keep record of all files and documents.
  • Orders and maintains all supplies needed for program.
  • Discloses lab results to clients in a confidential and safe manner.
  • Understands and implements operating policies and procedures.
  • Collects and processes specimen samples in accordance with National, State, and Local guidelines.
  • Ensures 100% of newly enrolled HIV-positive clients are offered PCRS through medical outpatient services, and periodically thereafter throughout a persons participation in clinical care.
  • Conduct field investigations to refer STD/HIV infected clients who are high risk for STD/HIV examination and/or treatment.
  • Works in collaboration with STDP is aimed at integrating STDP for screening, and reducing the number of new infections by adapting and implementing a community based models.
  • Provides or secures training or technical assistance to medical clinic staff to ensure maximum understanding and utilization of available PCRS services.
  • Conduct formal presentations with Health Care staff, CBO’s, groups of potential clients and other groups about PCRS.
  • Locates, informs and counsels, and provide referrals for testing/examination to all sex and needle sharing partners of STD/HIV positive clients identified through case management activities by AHF programs.
  • Conducts investigations to locate cases and contacts of sexually transmitted disease and HIV by interviewing clients to determine contacts, locating and tracing contacts, and convincing them to voluntarily seek diagnosis and treatment.
  • Counsels individuals who suffer from a STD/HIV regarding the nature and control of their disease and educates these individuals on risk reduction techniques and behaviors.
  • Distribute PCRS program materials and conduct outreach about the PCRS program.
  • Conducts HIV Pre and Post Test Counseling.
  • Provide case management services to STD and HIV positive individuals and their sex/needle-sharing partners.
  • Ensure that all preliminary HIV-positive persons arrange for a confirmatory HIV test and results disclosure.
  • Ensure that all HIV-positive persons receive appropriate client-centered health and social service referrals, consistent with STDP-mandated standards of care.
  • Serve as a liaison between the agency and STDP PHI Coordinator to meet all STDP-mandated evaluation and reporting requirements and maintain program evaluation indices.
  • Meet quarterly or as needed with the STDP PHI Coordinator and STD Program representatives to ensure quality assurance, review program practice, share best practices and areas of challenge and share strategies for demonstration project refinement.
  • Serve in a leadership capacity in the program by assisting the PHI/PCRS community Liaisons when necessary and by taking initiative to ensure the program meets its goals.

Supervisory Responsibilities:               

Supervises staff in the field as required.
