The Linkage to PrEP Specialist will:

  • Facilitate Center outreach, linkage, education, resources and services regarding Pre- and Post-Exposure Prophylaxis as an effective HIV prevention method.
  • Conduct targeted PrEP outreach to high-risk individuals.
  • Provide navigation and care management services to link clients to access PrEP, assist to remain compliant, and to retain in care.
  • Will provide in-service educational presentations to outside organizations and groups to increase awareness of PrEP/nPEP locally.

Priorities

  • Provide basic non-clinical linkage-to-care and patient navigation for individuals seeking PrEP/nPEP as a practical prevention tool
  • Conduct targeted outreach—both traditional and internet—on PrEP/nPEP to high-risk individuals among diverse populations, including gay, bisexual and other men who have sex with men (MSM); transgender individuals; and communities of color
  • Provide in-service presentations/outreach presentations on PrEP/nPEP to other organizations/groups/events. Maintain presentation materials and participant sign-in sheets
  • Maintain and update local PrEP Provider Directory and website of culturally-proficient PrEP services available to the community, including competent providers for transgender and MSM communities
  • Enroll, and provide care management services to clients to truly link to access PrEP, assist to remain compliant to PrEP, and/or to retain in care
  • Work with individuals to assess needs, develop care plans and coordinate PrEP-related services through advocacy, education, support, empowerment and coaching
  • Engage high-risk individuals in their neighborhoods or venues where they typically congregate to provide PrEP/nPEP-related health information, outreach, education, referrals and linkage to care services
