A Lead Medical Case Manager contributes to AHF by providing a range of client-centered services focused on improving health outcomes in support of the HIV care contimuum. Using this client centered focus, the LMCM will promote and support client independence and self sufficiency through services that include treatment adherence counseling, coordinator and follow up of medical treatments, client advocacy and referrals to needed services. Your goals as the LMCM is to ensure that the clients with HIV/AIDS have timely access to comphrehensive medical care and essential support services to prevent disease transmission and delay the progression of HIV. The LMCM, in addition to maintaining an active case load, provides supervision to ensure consistency in services, documentation, department processes, and procedures. They develop and maintain consistencies across the CM department, ensuring optimum utilization of resources and driving department speicifc improvement initiatives. You will act as the point of contact to discuss program changes, progress, outcomes and evaluation. You will lead outreach efforts, including but not limited to the promotion of program services, supporting CM member’s in house visits if needed, collaborative activities and internal/external meetings and trainings. You will oversee enrollment referrals and time sesnstive intakes.

Licenses/ Certifications: Bachelor's Degree,  Associates Degree or equivalent number of years of expereince.
