Essential Duties & Responsibilities Includes the Following:

  

Case Management Duties:

  • Provides one-on-one social and emotional support to clients by understanding and utilizing SMART goals objective in developing a navigation plan with clients.
  • Consistent with case management standards of care, performs intakes, assessments of client needs and case management of immediate and short-term needs; evaluates information necessary to determine eligibility for entitlement programs and available services.
  • Assists clients to navigate the medical care system.
  • Schedules client appointments consistent with the client’s needs.
  • Coaches clients in adherence skills.
  • Facilitates communication with providers to identify primary care needs and assists clients in making connections with health care services and necessary follow-up.  Participates in case management and individual supervision sessions with clients.
  • Provides mobile clinic and drop-in service, crisis intervention and back-up to staff as needed; utilizes skills to assess mental health crises. 
  • Works with other service provider agencies, hospitals, clinics, drug and mental health treatment centers, legal services and programs that serve the priority population to ensure continuity of services.
  • Works with clients to identify gaps in adherence to medical and medication routines as prescribed by their clinician, if applicable. Collaborates with treatment peer advocates.
  • Advocates on client’s behalf and refers cases to other community resources when appropriate.
  • Maintains concise documentation and program notes in the client records according to departmental standards using a computerized information system.
  • Assists clients in understanding and applying for public and private benefits.

Participation in Meetings/Committees

Participates in case presentations, seminars, supervision, meetings, trainings, peer review as needed

 

Supervisory Responsibilities

This job has no supervisory responsibilities.

 

Minimum Qualifications

  • BA in social work, psychology, counseling or equivalent required with at least 2 years of case management or counseling experience in a social services/health care setting preferred
  • Functional knowledge related to HIV/AIDS and working with the LGBTQ+ community is a plus
  • Must be comfortable working in community settings.
  • Willingness to working collaboratively, but with the capacity to work independently
  • Must have a car, valid CA driver’s license and auto insurance
  • Experience in working with human services provision at community agencies, healthcare settings etc.
  • Strong communications skills: Exceptional oral and written communication skills including the ability to compose engaging messages, present data in a clear manner as well as an ability to research and analyze information
  • Driven by Accomplishment – Clarity on project goals and the personal drive to accomplish goals; finished projects are thorough and delivered on time. High level of personal accountability for the quality and impact of work 
  • Ability to work with others on the startup of a new program
  • Attention to Detail: Must demonstrate a strong ability to be attentive to small details; documents and materials are well thought out, accurate and clear; database entries are organized and up to date
  • Ability to form empathic and collaborative relationships with diverse clients and have flexibility in meeting their needs
  • High Level of Professionalism – A high level of professionalism in work and interpersonal interactions with both internal and external contacts, presents to others as organized, informed, and friendly.  
  • Availability for occasional evening and weekend work and some travel as needed. 

 

Other Skills & Abilities/Qualifications 

  • Sensitivity to issues surrounding HIV and AIDS.
  • Experience with motivational interviewing preferred
  • Knowledge of criminal justice systems, substance abuse, and mental health is preferred. 
  • Computer Skills -- Proficiency with Microsoft Office applications including MS Word, Excel, Access, and PowerPoint, Project and Outlook. Experience with database systems 
  • Sensitivity to multi-cultural and lifestyle issues
  • Strong organizational/prioritizing skills in a rapidly changing environment
  • Establish and maintain good working relationships with staff, medical providers and outside professionals; and maintain close communication with related health, educational, and social service agencies
