A Care Worker contributes to AHF by utilizing a strong experience in care coordination and social work. The Care Worker has an understanding of the social determinants of the HIV population, the challenges and barriers to being able to follow through on a plan of care. Reaching and talking to their patients, the Care Worker draws upon their experience with a caring, compassionate, and knowledgeable focus. Combined with a thorough understanding of the Ryan White Program within their specific area, the Care Worker collaborates with their Care Management team, Healthcare Center Staff, and Providers so that every Ryan White patient receives the support they require and access to available resources. This will include providing education, screening, and assistance, so that eligibility for the program is understood and achieved. Working directly within the Healthcare Center, the Care Worker reviews charts and progress notes, so contact with the patient is meaningful and maximum assistance is available. Establishing routine contact with patients based on their needs gives the Care Worker the unique ability to know their patients to work to remove barriers to carrying out their Service/Action/Care Plan goals.
A Care Worker enhances the reputation of AHF by utilizing your care management experience, you will meet our patients where they are, identify strengths and opportunities for growth, and assist them in reaching their goals. With your collaboration and engagement skills, you will bring the essence of the Ryan White Care Act - the safety net for uninsured HIV/AIDS patients - to our most vulnerable population. Your contacts with patients directly contribute to AHF’s success in meeting the deliverables of our Ryan White contract. Beyond the scope of the contract, your work contributes to measures established by the HIV/AIDS Bureau (HAB) Measures and the Department of Health and Human Services.

Licenses and Certifications: Bachelor’s degree in Social Work, Counseling, Psychology, Marriage and Family Counseling, and Human Services with a minimum of 1 year of Case Management experience. Knowledge of Ryan White Services is preferable. |
