The EHE Retention Specialist (RS) blends the mission of AHF with experience in navigating care, beginning with client access, linkage to care and retention. As a Retention Specialist, you will work with newly diagnosed clients, clients lost to care, and clients currently in care providing them with your experience in the healthcare system. Your compassion and ability to relate to clients provide a level of comfort and trust to our patients.
Combined with a thorough understanding of the Ending HIV Epidemic Program in San Bernardino, the RS will work closely with the Care Management team, Healthcare Center Staff and Providers so that every client receives the support they require and access to available resources. This will include providing education and health literacy. Working directly within the Healthcare Center, the Retention Specialist is available to talk with every client as needed.
By utilizing your experience, you will meet our clients where they are, identify strengths and opportunities for growth and assist them in reaching their goals. With your skills of collaboration and engagement, you will bring the essence of the Ryan White Care Act - the safety net for uninsured HIV/AIDS clients - to our most vulnerable population. Your contacts with patients directly contribute to AHF’s success in meeting the deliverables of our Ending HIV Epidemic contract. Beyond the scope of the contract, your work contributes to measures established by the HIV/AIDS Bureau (HAB) Measures. The client discussions and relationships forged by the RS work each step of the way towards understanding, viral suppression, appointment and medication adherence, better health outcomes, and quality of life.
Some additional qualifications include being bi-lingual in English/Spanish due in part to having a large Hispanic clientele base. Our candidate should have a minimum of 1 year case management experience in a healthcare setting. Having retention specialist experience is a plus along with mental health experience to vulnerable populations. Having a strong customer service background is important for this role as well.
