YOUR CONTRIBUTION TO OUR SUCCESS!

A Care Manager Assistant contributes to AHF by conducting proactive outreach to clients by providing education and assisting with gathering eligibility documents for the Medical Care Managers to complete the Ryan White eligibility of the program. The Care Manager Assistant's main focus is to support Care Managers in enrolling eligible clients in private insurance via ACA to ensure the Ryan White Program remains the last payor source. In addition, the Care Manager Assistant will also support clients needing Insurance Support Services, verify service eligibility, request invoices, process premium payment requests, data entry, and upload supporting documentation on time. The Care Manager assistant is responsible for tracking all clients enrolled in Insurance Support Services to ensure insurance policy receives payments on time. The Care Manager Assistant helps to resolve any issues to prevent the cancelation of policies. With the understanding of the Ryan White Program, you will work directly with the Healthcare Center and the Care Managers to support enrolling newly diagnosed patients, patients lost to care, and clients currently in care by collecting eligibility documents for the Ryan White Program; as well as to support with ACA enrollments, and processing of Insurance Support Services to ensure clients maintain access to their HIV care continuum. In this way, you will support clients in accomplishing goals from the plan of care established by the care managers; your support with these activities will enhance the client's ability to maintain and improve their health status so they can work to achieve their healthcare goals.

A Care Manager Assistant enhances the reputation of AHF by making all necessary efforts to ensure clients have access to private insurance when eligible and that clients are appropriately screened for Ryan White Eligibility under all other pertinent third-party benefits programs. You will meet our clients where they are, identify strengths and opportunities for educating them about accessing and maintaining benefits, and assist the Care Managers in accomplishing all realistic goals. With your skills of collaboration and engagement, you will bring the essence of the Ryan White Care Act - the safety net for uninsured HIV/AIDS patients - to our most vulnerable population. Your advocacy to ensure our clients have access to private insurance and the Ryan White Program directly contributes to AHF’s success in meeting the deliverables of our Ryan White contract. Beyond the scope of the contract, your work contributes to measures established by the HIV/AIDS Bureau (HAB) Measures and the Department of Health and Human Services.

You will be a passionate advocate for our top initiatives.

To be successful as a Care Manager Assistant it is crucial to ensure that the AHF Core Values and Mission stay top of mind with all that you do. At AHF we are nimble and able to adapt in a dynamic environment to assist in providing the best experience for our clients and workplace for our employees.

AHF has a collaborative organizational structure where staff are accountable to multiple leaders.

The Care Manager Assistant will work as a part of a close-knit team to collaborate with the Care Management team and healthcare Center staff so that every Ryan White client receives support with enrollment to private insurance, Insurance Support Services, and the Ryan White Eligibility assistance they require, as well as access to available resources.

Licenses and Certifications: High school diploma or general education degree (GED) required.
