A Retention & Re-engagement Specialist contributes to AHF by supporting the Department of Medicine in achieving its retention goals. This role works closely with providers and support staff to recognize patients at risk for falling out of care, works with newly diagnosed clients to encourage participation in their care and identifies clients who have fallen out of care and reengage them.
A Retention & Re-engagement Specialist enhances the reputation of AHF by serving as a liaison to the community, as a resource and an additional touch point for the client.
