A Healthy Housing Care Manager contributes to AHF by assisting clients with access to resources, working with potential and existing tenants to remove barriers by facilitating a collaborative process that identifies client’s needs, a plan to address those needs, and continuous follow-up to ensure that members obtain the necessary services that help them to maintain successful residency. The HHCM coordinates with other service providers through a comprehensive and active referral process to link appropriate services and care to clients.

 

 A Healthy Housing Care Manager enhances the reputation of AHF by utilizing your strong skill set and a caring and mission-driven focus, you will work with Healthy Housing Foundation residents, external agencies, area housing coalitions, and other related resources to implement, support, and monitor the housing case management needs of the residents. You will serve as a key component in improving the resident’s stable housing options focused on successful residency and improved quality of life. The Housing Care Manager will provide CM services onsite at the Healthy Housing location and will also provide CM services to at least 25 tenants under the Scatter Site Housing CM program. The Care Manager to complete at least 2 home visits per month in the home of the tenant to ensure they are well adjusted to their home environment and to address any needs of challenges that may arise. Minimal travel expected at least 50% from tenant’s housing unit, to area resources, and various agencies required to meet the client’s needs.

 

You will be a passionate advocate for our top initiatives. To be successful as a Healthy Housing Care Manager, you will need to become familiar with Housing Forward’s Intake, Recertification, and Exit process, thoroughly document every encounter with client and made on the client’s behalf, as it is crucial to ensure that the AHF Core Values and Mission stay top of mind with all that you do. At AHF we are nimble and able to adapt in a dynamic environment to assist in providing the best experience for our clients and workplace for our employees. AHF has a collaborative organizational structure where staff are accountable to multiple leaders. The Healthy Housing Care Manager will work as a part of a close-knit team to develop a comprehensive database of resources designed to address client needs, including initiatives that incorporate wrap-around services that are equitable, inclusive, and tailored to the needs of individuals and families. The HHCM coordinates with other service providers through a comprehensive and active referral process to link clients to appropriate services and care.

 

Licenses and Certifications:

BS/BA in Social Work, or other social service field with two (2) years of direct service delivery.

Familiarity with housing case management.

Familiarity with housing entitlement systems and procedures. HOPWA Training Modules
