A Housing Care Manager contributes to AHF by assisting clients with access to resources, working with potential and existing tenants to remove barriers by facilitating a collaborative process that identifies client’s needs, a plan to address those needs, and continuous follow- up to ensure that members obtain the necessary services that help them to maintain successful residency. The HCM coordinates with other service providers through a comprehensive and active referral process to link appropriate services and care to clients.
A Housing Care Manager enhances the reputation of AHF by utilizing your strong skill set and a caring and mission-driven focus, you will work with AHF clients, external agencies, area housing coalitions, and other related resources to implement, support, and monitor the housing case management needs of the clients. You will serve as a key component in improving the resident’s stable housing options focused on successful residency and improved quality of life.
