Required:
Bachelor’s Degree in Human Services, Psychology, Sociology or Social Work related field and 1-2 years’ experience in the field of social work/case management or similar field.
JOB SUMMARY:
The Support Services Manager-Housing is a frontline leader that provides direct leadership, supervision, and support to the Department of Member Services staff for the purpose of ensuring the objectives and deliverables of the department are met. Additionally, the Support Services Manager-Housing works directly with members to remove barriers to HIV/AIDS medical care by facilitating a collaborative process that identifies members’ needs, a plan to address those needs, and continuous follow-up to ensure that members obtain the necessary services that help them to maintain adherence and retention to medical care.
The Support Services Manager-Housing ensures that applicants and participants of AID Atlanta’s financial programs are receiving accurate housing information and appropriate resources needed to help them establish and maintain a successful housing plan. This may include informational sessions in the community, workshops and educational programs for recipients or potential recipients of AID Atlanta’s TBRA or other Financial Services programs. This may also include development and implementation of training for staff.
ESSENTIAL DUTIES & RESPONSIBILITIES:
o Become knowledgeable of Federal, State and local housing rules and regulations and programs to include rental assistance programs for low-income households.
o Develop and/or assist in the development, monitoring, implementation and adherence of program policy, procedures and standards of care for the purposes of maintaining consistency and accountability.
o Establish, monitor and manage allocated budgets by recording and tracking expenditures monthly as required.
o Assist the Associate Director of Housing in monitoring the activities of other staff, caseload coverage, and quality control inspections.
o Prepare all mandated reporting as required by supervisor, contract agency and funding agencies.
o Prepare and submit required monthly, quarterly and annual reports for grant purposes or for submission to the Associate Director of Housing.
o Prepare and submit information and statistics to assist in grant writing processes for obtaining government, foundation and unrestricted funding for Member Services programs.
o Track and report on program and member outcomes based on agency’s Key Performance Metrics.
o Prepare for and participate in grant funding monitoring site visits (Ryan White, Part A, EHE, and HOPWA)
o Develop, implement and employ ongoing training for staff to increase their level of competence.
o Asses the effectiveness of referral agencies and provide feedback to Associate Director of Housing as appropriate.
o Interface with other HOPWA funded agencies to ensure services are not duplicated.
o Work in collaboration with The Housing Team to access housing related resources.
o Build partnerships, communicate regularly and maintain appropriate working relationship with onsite and off-site service providers/partners to ensure ongoing effectiveness of site-based services and effective collaboration communication between service provider and onsite/off-site staff (including attending onsite/off-site meeting making site visits, etc.).
o Attend and actively participate in various local, community, state-wide, national meetings, committee and workgroups (e.g. Ryan White, EHE, HOPWA, HRSA, DPH, Business Advisory Councils, Community based agencies, etc.) to build partnerships between agencies, add resources and impact local community policies.
o Conduct presentations about services offered including Housing case management and other AID Atlanta programs/services to various professionals/agencies as required.
o Employ continuous quality improvement strategies to enhance service outcomes including performing audits for quality purposes.
o Adhere to policies and procedures for AID Atlanta and other off-site programs to ensure quality standards are met.
o Network & meet with providers, representatives from ASOs and other community providers that may have contact with potential participants in the support programs of AID Atlanta.
o Provide direct service coordination and support to people living with HIV/AIDS in accordance with Case management standards of care.
o Record and document all interactions with clients.
o Provide services to Individuals and families that are program participants or seeking case management services on a walk-in, scheduled, and on-call basis.
o Assess and evaluate members’ initial needs through the HOPWA screening tool.
o Educate members about case management, and policies (grievance, HIPAA, member’s rights & responsibilities) and other agency program policies to ensure member has clear expectations of services to include housing and medical related services.
o Advocate and mediate on members’ behalf to decrease or eliminate barriers to care and housing services.
o Educate members about the importance of actively participating in their housing plans to ensure housing stability.
o Educate members about the importance of participating in medical care and ensure that members have full access to primary medical care and medication.
o Assess and Educate members about the importance of medication and medical adherence to improve member’s clinical outcomes.
o Assess and educate members about harm reduction, prevention, and other HIV-related sexual transmitted infections (STIs).
o Conduct home and hospital visits, accompanying members to appointments as needed.
o Organize and facilitate new member orientation to ensure members have a clear understanding of how Case Management can be utilized beneficially.
o Assess, verify and maintain records of a member’s eligibility for Case Management and various entitlement programs.
o Assess, collect, and maintain accurate member information and records in a confidential manner.
o Complete monthly reporting to ensure accurate programmatic reporting (based on position/site assigned).
o Provide active referrals and application assistance and support to members.
o Educate members about various community resources and programs to increase their knowledge of appropriate referrals.
o Collaborate with physicians, nurses, and other clinical staff in the development and execution of the members’ care plan where applicable.
o Attend monthly interdisciplinary meetings with medical providers (based on-site).
o Attend monthly individual supervision, group meetings/huddles, with Associate Director of Housing.
o Attend department and agency-wide meetings.
o Maintain up-to-date resources of community services.
o Attend regular and mandatory training (including Member Services, Ryan White, EHE, HOPWA, and others as assigned).
o Complete all HOPWA, HealthStream and required member services training annually or as assigned.
o Participate in various mandatory trainings held internally and externally ensuring a continuous knowledge base of programs that benefit members.
o Attend agency, local, state, and national meetings, conferences, or workshops as needed or required.
o Participate in and volunteer for intra and interdepartmental activities/events across the agency (i.e. AIDS Walk fundraising, agency sponsored events, member centered events, etc.).
o Communicate regularly with managers about changes, updates, and/or improvement suggestions in service delivery issues or other agency-related issues that directly or indirectly impact staff and/or members.
o Adhere to policies and procedures for AID Atlanta and other off-site programs to ensure quality standards are met.
o Abide by NASW code of ethics, HIPAA and Atlanta EMA Case Management Standards to ensure a high level of professionalism is maintained.
o Address members concerns utilizing the Customer Service Standards and grievance procedures.
o Meet with supervisor at least monthly to discuss chart audits/observation results, and to discuss performance and progress of stated goals.
o Other special projects, duties and responsibilities, as needed.
SUPERVISORY RESPONSIBILITIES:
o Provide direct support and supervision for Housing Case Management and Housing Information and Resources staff for the purpose of ensuring that the objectives and deliverables of the department are met.
o Provide day-to-day support, guidance and encouragement to housing staff for the purpose of ensuring effective service provision for members.
o Provide regular and appropriate feedback including training, opportunities for professional growth, verbal, and written improvement plans when needed and regular evaluations.
o Conduct and document monthly supervision sessions with staff to address member cases, discuss chart audits/observation results, and to discuss performance and progress of stated goals.
o Ensure that non-medical support staff are adhering to job duties/responsibilities and provide guidance and support as needed.
o Ensure that compliance with agency standards and grant requirements and contractual agreements are met by each staff member.
o Conduct mandatory chart audits, member record reviews, observations, and other duties as outlined in the Supervisory Standards.
o Communicate regularly with staff about changes, updates, improvements etc. in-service delivery issues or other agency-related issues that directly impact direct reports and/or members.
o Prepare and facilitate group meetings with direct reports to keep them abreast of changes of policies, build a team that supports one another in servicing members’ needs and encourages development of new skills/competence.
o Recruit, hire and train qualified staff to fulfill Member Services program duties as assigned.
o Conduct probationary employee performance evaluation/review with direct reports for the purpose of providing feedback on their observed strengths and areas of improvement.
o Handle Administrative Human Resources duties including tracking of timesheets, employee leave, absences vacation, staff coverage, etc.
o Address disciplinary actions of direct report staff and advise Associate Director of Housing of intended action implemented to improve situation.
o Respond, intervene, and address member and/or staff issues (complaints, threats, emergency financial situations, etc.) in a timely manner utilizing Customer Service Standards.
o Abide by NASW code of ethics, HIPAA and Atlanta EMA Management Standards ensure a high level of professionalism is maintained.
o May be required to perform the functions of other housing staff during times of vacancies.
QUALIFICATIONS:
EDUCATION & WORK EXPERIENCE
o Bachelor’s degree in social work, Psychology, Sociology, Human Services or social services-related field.
o 1-2 years’ experience in the field of social work/case management or similar field.
o Some supervisory experience preferred
o Experience in the field of HIV is preferred, but not required
o Must pass criminal background check.
