An Associate Director of Care Management South Central contributes to AHF by providing both clinical and managerial oversight of Care Management programs and contracts. This position requires a strong understanding of Ryan White Care Management's perspective and a thorough understanding of each program’s guidelines, HIV/AIDS Bureau (HAB) measures, standards of care, and available resources. You will cooperate with the South Central Regionals, Leadership Team and Healthcare Center team to ensure that your teams understand the operational process, deliverables, and performance goals of the Care Management Division and each Ryan White program. You will provide clinical guidance and operational assistance to your team. You will monitor and analyze care management productivity and performance reports to identify trends, best practices, and challenges. By conducting 1:1 meetings with your Managers/Supervisors, you will understand the barriers and challenges, and with a solution-oriented focus, you will ensure collaboration with all of AHF.

 

 An Associate Director of Care Management South Central enhances the reputation of AHF by conducting internal audits of each program, ensuring that supervisory duties are handled in a satisfactory manner and according to AHF and grantee policy and procedure, and report summaries of each program, successes, challenges, and trends to your direct report to the Senior Director of Care Management Grant Programs. You will also offer assistance as the Professional Resource for the HCCs in the Care Management program. As Associate Director, you will collaborate creatively to meet contract and program expectations and deliverables and improve processes. You will bring the essence of the Ryan White Care Act - the safety net for uninsured HIV/AIDS patients – to all programs under your oversight and keep open and ongoing discussion and understanding with your team. This role plays a significant part in facilitating required Quality Improvement initiatives by working closely with the QI Managers to establish improvement activities within staff workflows. You will review the deliverables so they remain on target. Other programs may be assigned by the Senior Director of Care Management Grant Programs or designee as required by business needs. Other regions may be assigned as appropriate for this role.

 AHF has a collaborative organizational structure where staff are accountable to multiple leaders.

The Associate Director of Care Management South Central will work as a part of a close-knit team to implements and oversees strong collaboration and cooperation with all divisions of AHF so that the client experience is optimum. You must communicate and cooperate with the South Central Leadership team. This position will bring a cohesive managerial approach to Ryan White programs and ensure that the AHF culture and standards of providing Care Management services to all AHF patients is front and foremost. The Associate Director of the South Central upholds the highest level of care to our patients by being available to troubleshoot issues, act as a liaison to other programs and departments, monitor progress to annual program goals, ensure fidelity to policies and procedures, and prepare reports in response to contract requirements, among other responsibilities. This position provide oversight of and guidance and support to the local supervisors in each region, to ensure that there are no barriers to providing a seamless client experience.

 

Licenses and Certifications: Current and good standing Licensed Clinical Social Worker degree or Licensed Registered Nurse from an accredited institution. Masters preferred. Masters in Social Work or a related field is required Minimum 5 years experience in the delivery of Ryan White Program services. 2 years of Management experience required. Ability to travel in your regions up to 50% of the time.
