Today, AIDS Outreach Center (AOC) began as a grassroots organization responding to the HIV epidemic. Since that time, AOC has been a leader in North Texas in the fight against HIV. Annually, 42 staff members provide services and education to over 2,000 people living with HIV and prevention services to over 3,500 people in our community.

GENERAL DESCRIPTION:
Reporting to Housing Program Manager, staff provides ongoing housing case management services to persons who are participants of the HUD HOPWA Housing Program and their significant family members. This position aligns with the U.S. Department of Housing and Urban Development (HUD) and Housing Opportunities for Persons With AIDS (HOPWA) program objectives by using a Housing First approach -removing barriers to housing entry and offering supportive services to promote long-term stability and wellness. Duties include: needs assessment, developing a comprehensive housing plan, coordinating services and advocating on behalf of each client. The Housing Case Manager functions as the key human link between the client and the service system within the community. Many needed services will be performed within the context of the client's natural environment rather than in an office setting.
PRIMARY RESPONSIBILITIES:
Develop comprehensive housing plans, establishing goals in collaboration with the client (including family, significant others, and other social service providers when appropriate), aimed at increasing the level of functioning and self-sufficiency in all areas of life.
Follow-up on referrals to agency and community service providers, as necessary, and coordinate activities in order to provide an integrated system of service delivery.
Review client’s level of functioning, update housing plans, and maintain client contact in accordance with the client’s need.
Provide informal counseling and support in areas such as: Activities of Daily Living, Use of Resources, and Coping Strategies.
Document service delivery and maintain client records and confidentiality in accordance with AOC/AHF Policies & Procedures.
Provide regular home visits and case management support focused on housing retention, connection to medical care (including HIV primary care), mental health and substance use services, income and benefits access, and other stabilizing supports.
Collaborate closely with landlords, property managers, community partners, and internal teams to address housing crises and reduce evictions.
Maintain accurate and timely documentation in accordance with HUD/HOPWA requirements.
Participate in coordinated entry processes and local Continuum of Care efforts to prioritize housing for the most vulnerable populations.
Adhere to principles of Harm Reduction, Trauma-Informed Care, and client choice.
Develop an effective working relationship with area clinics and doctors to communicate appointments with clients as appropriate, and forging relationships with community partners within the scope of the program.
Report client service statistics; attend and participate in agency related meetings, retreats, work sessions, and events as requested; perform other related duties as assigned by the Housing Program Manager.
Complete required HUD HOPWA trainings within 1 month of hire date. Complete 15 hours of job related CEU or other trainings per year.
QUALIFICATIONS:
Bachelor's degree in social work; minimum licensure for level of education obtained within 12 months of hire date; knowledge of HIV/AIDS issues; knowledge of grief and bereavement; knowledge of medical treatments available for HIV/AIDS; knowledge of family systems; knowledge of cultural/ethnic issues; knowledge of addiction process and behavior; knowledge of community resources; excellent written and verbal skills; adhere to Social Work Code of Ethics; participate in ongoing continuing education activities; knowledge of gay, lesbian, bisexual and transgender issues; ability to work with diverse populations; ability to complete assigned tasks in a timely manner; ability to effectively handle crisis/emergency situations; demonstrates a non-judgmental attitude toward clients, staff and volunteers; ability to discuss sex/sexuality issues in an appropriate manner with clients; ability to adapt quickly to change; ability to build rapport and utilize a holistic approach to change; ability to use a strengths-based perspective when working with clients; ability to establish and maintain appropriate boundaries; proficient in Microsoft Office; valid Texas driver's license reliable transportation, and willingness to conduct field work throughout Tarrant County.
