A Community Health Worker contributes to AHF by utilizing a strong experience in care coordination with patient access, engagement, and retention. Your goal will be to assist in keeping clients in care. You will support clients who are at risk of nonadherence. You will assist them in navigating the healthcare system and achieving positive healthcare outcomes. You will also plan, coordinate, and implement specific interventions to find and re-engage patients lost to care. Utilizing your community resource experience and a strong skill set in understanding and working through the healthcare system, you will work with new clients, those at-risk members, and those who are lost to care and support them with the ever-changing healthcare system. Your compassion and ability to relate will comfort and trust the clients with whom you provide this care, engagement, and access to resources.
A Community Health Worker enhances the reputation of AHF by Understanding the locality and the specific challenges facing this population to find creative solutions to re-engaging members at risk of, or have been lost to care, will be pivotable in this role. Keeping patients engaged in and understanding their healthcare is the goal of the Community Health Worker so that healthy behaviors are achieved, resulting in a healthier population. With your creative skillset, you will work to achieve stronger engagement and health outcomes. Beyond the scope of the contract, your work contributes to measures established by the HIV/AIDS Bureau (HAB) Measures and the Department of Health and Human Services.

AHF has a collaborative organizational structure where staff are accountable to multiple leaders.
The Community Health Worker will work as a part of a close-knit team to Have an understanding of the Ryan White Guidelines/Standards; you will collaborate with the Medical Care Management team, the Healthcare Center, Provider(s), and team and outside resources so each client receives access to available resources and the support required to be successful with their medical care.
Licenses and Certifications:
High School/equivalent diploma required.
