Today, AIDS Outreach Center (AOC) began as a grassroots organization responding to the HIV epidemic. Since that time, AOC has been a leader in North Texas in the fight against HIV. Annually, 42 staff members provide services and education to over 2,000 people living with HIV and prevention services to over 3,500 people in our community.

Our Mission is to serve people living with or at risk for HIV with dignity and respect.

In our community, people living will with HIV receive quality care, and new infections are rare.

GENERAL DESCRIPTION:

Reporting to Director of Community Health, the position oversees the Transgender Wellness Program and Mpowerment project and works closely with Community Health Department and project volunteers to ensure the fulfillment of both program goals. Will work with Director regarding budgetary and finance issues, including the timely handling of invoices and receipts in accordance with polices.

 

PRIMARY RESPONSIBLITIES:

Mpowerment Project/Transgender Hormone Therapy Clinic

Reports to Director of Community Health with updates regarding programing for Mpact, troubleshooting any issues with both programs, and help with all Community Health events.  

Works with Community Health Team to ensure the fulfillment of all team projects.

Orders material for clinic and project space and safer sex promotion materials.

Will assist with training Peer Advocate to develop MPact Calendar, design program activities & implement outreach & recruitment for Mpowerment project.

Assist Peer Advocate with M-group recruitment and M-group sessions.

Assist peer advocate with submitting monthly reports, specifically on outreach contacts, drop-in numbers, Core group attendance, M-group event attendance, informal outreach.

Coordinates Large events, small weekly events, and community collaborations.

Coordinates all facets of the transgender hormone replacement therapy program.

Recruits participants for the clinic and other programs through a variety of outreach activities.

Works with the nurse practitioner (provider) and sets clinic schedules and conducts clinic activities.

Conducts oneself professionally and in a culturally sensitive manner at all times.

Provides Trans clinic intake with each client, including obtaining medical, sexual, and other information and explaining the different aspects of the Trans program.

Establishes, tracks, and maintains clinic client appointments and communication.

Submits and tracks lab requisitions prior to each Trans clinic day.

Manages inventories and tracks clinical supplies.

Prepares clinic charts with labs and any necessary documentation for clinic days.

Greets clinic clients and updates client information upon their arrival or call.

Conducts pre-exam patient vitals to ensure nurse practitioner sees clients on time.

Provides education regarding medication dosing, administration, and side effects.

Liaises communication between client and nurse practitioner, as needed.

Transmits prescriptions and refills to pharmacies as directed by nurse practitioner.

Troubleshoots pharmacy issues on behalf of clients regarding dosing or other medication questions.

Provides clients with information on gender marker services.

Maintains Trans program clinic charts for client follow-up.

Follows best gender care best practices and manages patient situations.

Troubleshoots lab, e-prescription, and other service issues, etc., as needed.

Performs other duties as required.

 

Programmatic

Conducts program outreach and educational sessions in a variety of settings.

Coordinates client referrals to behavioral health and other services, as needed.

Tracks, monitors, and maintains essential supplies.

Work with different databases.

Drives mobile testing unit, as needed.

Provides client phone consultation as needed.

Attends department, agency, and other meetings, as required.

Fulfills all other tasks as assigned.

 

Other Duties: Performs all other duties assigned by supervisor/director.

Lifting Requirements:   At least 20-50 pounds.
