A Health Navigator contributes to AHF by  providing effective and efficient facilitation of linkage navigation services for racial and ethnic minorities in Chicago.  

 

A Health Navigator enhances the reputation of AHF by providing compassionate and informed care to our clients.  These duties allow the opportunity to engage within the community and educate those at risk.  Caring, personable and relatable team members enhance the probability of clients returning for services in the future.

AHF has a collaborative organizational structure where staff are accountable to multiple leaders. 

 

The Health Navigator will work as a part of a close knit team to provie linkage to care activities, including accompaniment, follow up services and peer support.  The HNS will also be responsible for providing HIV, STI, and HCV testing or referrals to these services. The Health Navigator works with other program team members to enhance the number of clients receiving services from AHF/CALOR. The Health Navigator will also assist the team in developing and conducting effective referrals to supportive services including housing, food, financial assistance and employment.
