YOUR CONTRIBUTION TO OUR SUCCESS!

A Licensed Manager of Care Management contributes to AHF by utilizing your experience in overseeing Care Management Services at a managerial level in your assigned EMA/TGA. With a strong understanding of the Ryan White services, you will be well-versed in contract deliverables, Standards of Care, and the goals of the program and the patients served. You will collaborate with the AHF Healthcare Team, including Providers, the Practice Manager, the Associate Director, and your contract manager to ensure your team understands the operational process and deliverables. You will provide clinical support to your team and monitor services entered into the Care Management Information system. You will ensure that programs under your oversight remain audit-ready and that your team has the necessary knowledge, training, and tools to implement your operational goals effectively. Through regular 1:1 meetings, you will assess team performance and work collaboratively with your team to resolve any challenges. In doing so, you will empower your team to help their patients achieve their healthcare goals.

 

A Licensed Manager of Care Management enhances the reputation of AHF by collaborating with the Associate Director of Care Management to lead clinical operations reviews, creatively address contract expectations, and ensure deliverables are met. You will bring the essence of the Ryan White Care Act - the safety net for uninsured HIV/AIDS patients - to the team for ongoing discussion and understanding. You will review the deliverables to ensure they remain on target. Beyond the scope of the contract, your team will contribute to the measures established by the HIV/AIDS Bureau (HAB) Measures and the Department of Health and Human Services.

AHF has a collaborative organizational structure where staff are accountable to multiple leaders.

The Licensed Manager of Care Management will work as a part of a close-knit team to add consistent value to the Healthcare Center by ensuring that all written policies and procedures align with the current EMA/TGA HIV Case Management Standards. This position will collaborate with both staff and leadership to implement the Model of Care, meet the required standards, and improve member care and outcomes. This position contributes to the Care Management Division's success by effectively managing the key initiatives within Care, as well as all applicable county, contractual, federal, and state regulatory requirements. Other Healthcare Centers (HCCs) may be assigned to this role as needed.

 

Licenses and Certifications: Current and good standing Licensed Clinical Social Worker or Licensed Master Social Worker in the jurisdiction in which services are rendered; A minimum of 3 years of Case Management experience is preferred; 2 plus years of management experience preferred; Ability to travel in your regions up to 50% of the time.
