A Housing Case Worker contributes to AHF by utilizing strong experience in Social Services and understanding the social deterrents of people living with HIV/AIDS and a thorough understanding of Social Service Programs within Cuyahoga County. With a caring and knowledgeable focus, you will work with all of the teams in the Cleveland office so that every patient receives the support they require and access to available resources.
A Housing Case Worker enhances the reputation of AHF by assisting patients with their housing and utility needs. This ranges from assisting a patient with rent arrears to avoid an eviction to working with patients to find permanent housing and/or providing utililty assistance. All of this is done while working to keep the patient in care.

The Housing Case Worker will work as a part of a close knit team to meet our clients where they are, identify strengths and opportunities for growth an assist them with realistic goal setting.
