A Licensed Clinical Care Manager contributes to AHF by utilizing strong social work skills and a deep understanding of the Ryan White Program to serve the clients in their area. The Licensed Clinical Care Manager (LCCM) focuses on serving older adults (ages 50 and above) living with HIV/AIDS and experiencing multiple psychosocial and health related needs. This will include providing health education, medical coordination, follow-up on medical treatments and patient advocacy. Working directly within the Healthcare Center, the LCCM will meet with clients and conduct Comprehensive Assessments, develop Care Plans, and provide ongoing education to help clients understand their diagnoses and medications. The goal is to support adherence to overall health through counseling and skill-building. You will provide advocacy and emotional support to reduce health disparities and conduct retention calls to ensure continuity of care for clients to prevent health disparities. This role upholds radical cooperation across divisions, ensuring no gaps in retention efforts and promoting integrated care delivery.

 

A Licensed Clinical Care Manager enhances the reputation of AHF by utilizing strong Care Management principles. You will meet our patients where they are, identify strengths and opportunities for growth, and assist them with realistic goal setting. With your collaboration and engagement skills, you will bring the essence of the Ryan White Care Act (the safety net for uninsured HIV/AIDS patients) to our most vulnerable population. By establishing Needs Assessments, Care Plans, referrals, social support, and other needed services, your work will directly contribute to AHF’s success in meeting the deliverables of our Ryan White contract. Beyond the scope of the contract, your work contributes to measures established by the HIV/AIDS Bureau (HAB) and the Department of Health and Human Services.You will assure the coordination, follow-up, and completion of medical treatments and/or care plan interventions so that these services ensure timely and coordinated access to medically appropriate levels of health and support services and continuity of care through ongoing assessment of the client’s and other key family members’ needs and personal support systems. You will verify the client’s eligibility for the ACA (Affordable Care Act) program and enroll those who qualify to ensure broader access to healthcare services.

AHF has a collaborative organizational structure where staff are accountable to multiple leaders.

 

The Licensed Clinical Care Manager will work as a part of a close-knit team to ensure clients have timely access to comprehensive medical care and social services, while promoting independence and self-sufficiency. The LCCM for older adults will assist clients in developing, achieving, and maintaining healthcare appointments, adhering to antiretroviral therapy (ART) and other prescribed medications, and coordinating care with healthcare providers for comorbid or chronic conditions. Additionally, the LCCM will support clients in maintaining adherence goals aimed at improving overall health through education, counseling, skill-building, and enhanced access to services within the continuum of care of HIV Health Services continuum. This work is conducted withing a primary medical care environment, ensuring integrated and patient-centered care.

 

Licenses and Certifications: Current and good standing Licensed Master Social Worker (LMSW) in the jurisdiction in which services are rendered. A minimum of 1 year of Case Management experience. Knowledge of Ryan White Services is preferable.
