As a Care Management Program Community Health Worker (CHW), you will work to create connections between our vulnerable patient population and their AHF Healthcare Center healthcare providers.

As a CHW you will be called upon to utilize your many unique talents to help patients navigate the healthcare and social service systems in various and challenging ways. The CHW assists with the transition of patients back into care, advocating for underserved individuals to receive services and resources to address health and social needs, documenting and collecting data to inform providers, the program and policies, assists the Care Coordination Team in building community relations to address patient needs and proactively addressing individual patient social determinants of health (SDOH) needs. As a CHW you will be a frontline agent of change in helping to reduce health disparities in our vulnerable population and increase their retention in care. By collaborating closely with medical providers, case managers, and other interdisciplinary staff, the CHW helps streamline the patient experience, reduce gaps in care, and strengthen overall health outcomes. This position is a key component of AHF’s broader retention and linkage strategy, ensuring that every patient receives consistent, compassionate, and coordinated care aligned with AHF’s mission and departmental performance goals.

You will convey the purpose and services of the Community Health Work to your patients, the HCC team, Medical Providers and the Care Coordination team. You will coach and assist patients in understanding and accessing community services and healthcare system support while incorporating AHF’s core values of patient first, respect for diversity, being nimble, fighting for what’s right and valuing your fellow employees and our AHF Mission: Cutting Edge Medicine and Advocacy, Regardless of Ability to Pay.
Keeping members engaged in and understanding their healthcare, coaching patients so they may improve their health outcomes, and assisting patients with insurance/benefit management are the goals of the Community Health Worker. Your compassion and ability to relate will provide a level of comfort and trust to our members.
As part of the Care Management Team, you will work with all patients of AHF, regardless of payer source, to provide support and access to services impacting their ability to achieve better health outcomes. Radical Collaboration is essential in this role as you will work across all divisions of AHF and the community at large so that patients remain engaged in care with successful health outcomes.
