This role involves collaborating with internal teams and external partners to monitor deliverables, maintain audit readiness, and ensure accurate documentation of services. By fostering collaboration and maintaining strong oversight, the Housing Program Manager empowers the team to deliver high-quality housing support and help clients achieve long-term housing stability.

AHF has a collaborative organizational structure where staff are accountable to multiple leaders.
The Housing Program Manager will work as a part of a close-knit team to hire, train and oversee staff supporting clients enrolled in AHF’s project-based and scattered-site Permanent Supportive Housing programs. The manager will ensure that team members understand DHA and Housing Forward intake, recertification, and exit processes to facilitate smooth transitions and continuity of care. This role includes guiding staff on accurate documentation of client interactions, maintaining updated care plans, and developing a resource database to support individualized services. Additionally, the Housing Program Manager will direct staff in coordinating with service providers for referrals, connecting clients to behavioral health, medical, and social supports, and identifying opportunities for formal partnerships through memorandums of understanding.
Licenses and Certifications: BS/BA in Social Work, or other social service field with two (2) years of direct service delivery. Familiarity with housing case management. Familiarity with housing systems and procedures.
