PHARMACIST - IMM Clinical Coordinator contributes to AHF’s mission by improving the clinical outcomes of our AHF Pharmacy patients through Medication Therapy Management (MTM).
The PHARMACIST provides a helpful voice, to our patients and customers seeking MTM services, clinical information and support for their medication adherence. This role will report out of our Hollywood clinic and work Monday-Friday 8 am to 5 pm.
In this role, you will provide a high level of clinical skills and knowledge to our patients that help them live a sustainable quality of life, be productive in society, and improving their health contributes to the success of AHF and our mission.
This PHARMACIST also reviews for medication problems, monitors medication adherence, reviews immunization history and provides interventions with patient and Primary Care Physician when necessary for improved pharmaceutical care. Contributes in ensuring the Clinical Patient Management Program runs smoothly in our home pharmacies and helps to further improve our patient’s health outcomes and contributes to the ultimate mission of AHF Pharmacy. Collaborative contributions are essential to our success and includes working with various departments within Pharmacy as well as other divisions of AHF, our patients and partners.
As part of your daily duties, the Pharmacist will play a key role in the MTM services including being a problem solvers and providing a “helping hand” to our patients and ensuring they understand everything about their medication including: What is it treating? How do I take it? What are the side effects?
