Summary:

Social worker assists HIV-positive clients to solve their social problems, to increse their adherence to treatment and thus to reach the best possible health outcomes.

 

Essential Duties & Responsibilities:

  1. To provide HIV-positive persons debt advisory services and mentoring;
  2. To provide HIV-positive persons with housing and housing-related services counselling;
  3. To provide HIV-positive persons in the job search counselling and assistance with CV compiling;
  4. To provide HIV-positive persons with educational training counselling and support counselling regarding raising qualifications;
  5. To cooperate with governmental agencies and organizations, including but not limited to the Estonian police, state social services authorities and the authorities of municipalities that provide social services;
  6. To provide HIV-positive persons with counselling and support in legal issues and how to resolve the encountered legal problems;
  7. Accompanying of HIV-positive persons in social services offices or local government authorities;
  8. To advise HIV-positive individuals how to apply for state and local government provided grants and pensions;
  9. Organizing document translation in connections of counselling of a HIV-positive individual;
  10. To keep contact with HIV-positive individuals, collection of data regarding their condition, as well as the encouragement of continuing treatment and in case discontinuance of treatment persecuting them to continue their treatment;
  11. If necessary, to compile necessary surveys, analysis and reports regarding fulfilment of the mandate;
  12. If necessary, to register the data of a HIV-positive with their prior permission, manage HIV positive individuals case related to mandate activities;
  13. Compile any necessary documentation, which is related to the performance of the mandate;
  14. To inform and deliver information to Linda HIV Foundation Management Board, Internal Controller or to the person appointed by the Management Board;
  15. To follow Linda Clinic internal regulations and internal rules;
  16. To inform the head of Linda HIV Foundation or Internal Controller of the violations of internal regulations and internal rules;
  17. To fulfil other duties and orders of the Linda HIV Foundation Management Board member or Internal Controller within the framework of the authorization agreement.
