Responsibilities

JOB SUMMARY:  
The Ryan White Medical Case Manager Florida Program   is responsible for projecting and integrating the Mission and Core Values of the AIDS Healthcare Foundation (AHF) in the provision of quality medical case management services to  AHF Healthcare center Ryan White eligible patients.

Under the direction of the Regional Director, Ryan White Medical Case Management, The MCM , shall adhere to a  proactive case management model intended to serve persons living with HIV with multiple complex psychosocial and/or health-related needs that focuses on maintaining HIV infected persons in systems of primary medical care to improve HIV-related health outcomes.

 

Essential Duties & Responsibilities
Duties and Responsibilities include, but are not limited to:

  • Key activities include (1) initial assessment of service needs; (2) development of a comprehensive, individualized service plan; (3) coordination of services required to implement the plan; (4) client monitoring to assess the efficacy of the plan; and (5) periodic re-evaluation and adaptation of the plan as necessary over the life of the client. It includes client-specific advocacy and/or review of utilization of services. This includes all types of case management including face-to-face, phone contact, and any other forms of communication.
  • Providing a range of client-centered services that link clients with health care, psychosocial, and other services.
  • Assure the coordination, follow-up and completion of medical treatments and/or care plan interventions so that these services ensure timely and coordinated access to medically appropriate levels of health and support services and continuity of care, through ongoing assessment of the client’s and other key family members’ needs and personal support systems.
  • Medical Case Management includes the provision of treatment adherence counseling to ensure readiness for, and adherence to, complex HIV/AIDS treatments.
  • Responsible to maintain a comprehensive knowledge base regarding the current standards of HIV/AIDS care and case management processes.
  • Provides education on HIV/AIDS and other co-morbidities including treatment adherence to medication regimens, including provider appointments, prevention, harm reduction, sexual health and preventative health measures, (Colorectal screening, PAP smears, PSA screening, etc.)
  • In conjunction with the patient, establishes long and short term goals for the patient/family which are S-M-A-R-T (Smart, Measurable, Achievable, Realistic, Timely)
  • Demonstrate the skills necessary to work with a variety of medical care professionals, including other medical case managers, and interdisciplinary personnel.
  • Conduct a timely intake and evaluation of each patient, completing the Common Intake Form (CIF) as part of this procedure
  • Collaborate to mutually develop and establish  an action or individual service plan with each patient that: identifies patient needs, goals, and planned course of action, promotes readiness for and adherence to complex HIV/AIDS treatments (i.e. counseling, special programs), and meets identified needs; monitors and documents the changing needs of the patient through personal contact and by reviewing the care plan at least every six months, revising it as necessary, and maintaining an original copy, signed by the patient (or including documentation that it was reviewed by the patient), in the patient record
  • Collaborate with Director to ensure that written policies and procedures are in place to describe how and under what circumstances the agency can discharge, terminate, or close a client’s record in accordance with the current Orlando FL HIV Case Management Standards.
  • Ensures that all case management related services (including but not limited to) case management notes, referrals, action plans, are recorded in the appropriate software in a timely manner in accord with the Orlando FL scope of work.
  • Maintains patient identifiable health information in a confidential manner and in accordance to AHF policy and HIPAA regulations.  Reports all breaches to supervisor.
  • Participates in the Quality Improvement program by collecting, analyzing and reporting on QI indicators on a routine basis.
  • Participates in local EMA functions in the community and reports back to management the content and actions of the various committees and groups.
  • Advocates for patient needs with Health Care Center, Medical provider, and Community.
  • Reports all complaints and grievances per grievance policy, assists in resolution when appropriate.
  • Maintains at least the minimum performance and productivity standards.
  • Assist AHF Grant department with collecting data in a timely fashion to ensure Invoices are prompt and correct
  • Attend all local Ryan White and Community Planning meetings
  • Conduct HIV Testing
  • Serve as the local contact person for this grant but coordinate all efforts with the Grant department.
  • Identify clients who need additional Retention in Care efforts and assist with ensuring they remain in care.
  • Work with AHF Grant staff to ensure all RSR data collection and entry is correct and complete.
  • Serve as a liaison between the community and AHF.
  • Other duties as assigned to ensure team goals are met

Education and/or Experience

  • Bachelors of Nursing Degree preferred.
  • Earned Bachelor degree from an accredited institution with a minimum of three years Case Management experience required.
  • Knowledge of community resources
  • Knowledge of target population
  • Knowledge of HIV disease and treatment
  • Competence in Cultural and linguistic Diversity.
  • Experience in care
