  • Conduct preliminary interviews/counseling for ART to selected clients so in order to assess client’s immediate needs, including medical, housing, mental health, transportation and other supportive services;
  • Measure client’s adherence level to routine medications such as septrin and ART and assess barriers to and supports for adherence;
  • Periodically reassess adherence level, barriers and supports, and make a record of all findings as well as plans generated during counseling;
  • Facilitate monthly support groups for HIV/AIDS treatment information, treatment adherence counseling, monitoring, and other strategies to support clients in readiness to begin ARV treatment or maintain maximal adherence to prescribed HIV/AIDS treatment.
  •  Conduct individual and group sessions to provide psychosocial support, general HIV/AIDS information and risk reduction activities, HIV one-0n-one, and partner counseling and referral services (PCRS) in addition to HIV/AIDS treatment information and treatment adherence counseling;
  • Conduct community outreach to locate clients who are out of care or need support to remain in care;
  • Identify resources and promote program; serve as liaison to community by conducting presentations on all program activities. Participate in community activities such as SEEP, health fairs, special events;
  • Assist with case management duties including accompanying client to referral appointments and case conferencing with service providers to ensure optimal service delivery to clients;
  • Track and record in ART and other relevant registers and program databases all activities undertaken;
  • Ensure the maintenance of appropriate, accurate, and clear documentation of all outreach, education, case management and group activities – report writing;
  • Assist program development and clinicians to explore for and identify additional linkage to Care services such as legal help, nutritional support; Identify resources and promote program; serve as liaison to community by conducting presentations on all program activities;
  • Actively participate in trainings, on-going skills enhancement trainings, and network meetings;
  • Establish a clear bi-directional process with medical care providers for the receipt and provision of relevant information on the medical and psychosocial status of enrolled clients;
  • Provide case management services including intake, needs assessment, service planning, service plan implementation, service coordination, monitoring and follow up, reassessment, crisis intervention, and case closure;
  • Assist in Rapid testing and prevention program activities and events upon request – such as trainings, testing events;
  • Participates in conducting needs assessments and recommending trainings based on gaps identified;
  • Represents AHF/UC in several forums and as assigned;
  • Undertakes any other duties as may be assigned.
