The Health Plan Nurse Practitioner (HPNP) blends the mission of AHF with advanced practice nursing skills to complete Medical Annual Wellness Visits and Acute Transition of Care Assessments. With a knowledgeable focus, you will enhance our patients care through strong clinical evaluation, documentation and collaboration with the member’s primary care provider. As a skilled clinician, you are a key contributor to telling our patients’ stories by clearly identifying the chronicity and severity of member’s health conditions. You will complete post hospital assessments, review for discharge plan adherence, and order additional treatments, medications, labs and procedures as needed to enhance clinical outcomes while reducing risk of readmission and preventing avoidable ED visits.
By utilizing core principles of nursing, you will meet our members where they are, identify strengths and opportunities for growth and assist them with realistic goal setting. With your advanced clinical skills, you will be part of our interdisciplinary team of RNs, LCSWs, Coordinators, Pharmacists and Doctors to educate, reinforce, rehabilitate, and support our members to heal and grow. Your assessments will directly contribute to the divisions review for medical necessity, under and over utilization, risk adjustment and linkage to primary and specialty care.
As HPNP, you will work with the Transition of Care RNs to direct the shift between levels of care where high acuity is involved. Your assessment and direction will help to re-establish members’ connection to their primary care provider, offer support and illness teaching and be a key factor in the member’s reengagement with their AHF health maintenance team. You will craft a plan of care to give the member vision and focus to heal. As an advanced practice nurse, we believe your talents will be key to our success as a department and to the success of our members’ health outcomes.
