Clinical-Information Management:

• Ensures maintenance of patient confidentiality.

• Huddles with assigned provider(s) and HCC staff at beginning of each day to discuss patients, staffing, etc. Meets with assigned provider at beginning of day and documents patient care activities to be completed and meets again with provider at close of day to discuss any open items and plans for follow-up.

• Completes information gathering on new patient including: family history, past medical and surgical history, current medications, social history and review of systems.

• Completes updates of all above patient information on provider follow-up appointments.

• Reviews ‘Protocol” form to determine tests/screening/vaccinations due during the patient’s visit. Prints the Protocol form for the provider’s review to ensure all protocols are up-to-date.

• Collects and documents patient's chief complaint, weight and height, pain scale ranking, temperature, pulse, respiration, blood pressure and pulse oximetry as required at patient healthcare visit as ordered or per protocol.


• Completes ‘chart scrubbing’ process including review of the patient’s EMR to ensure that all patient results/reports for tests, procedures and referrals are present in the EMR—this process is to be completed pre-clinic visit. All referral and procedure results to be scanned into the patient’s EMR with standard labeling for easy identification.

• Reviews the Missing Charges report weekly as provided by the Nurse Manager and reviews, corrects and completes documentation to ensure appropriate billing and documentation for provided services.

• Documents patient services performed--nursing documentation to be completed the day of service or within 24 hours for compliance and reporting purposes.

• Documents observations in the patient's medical record as requested by a physician/provider and/or Nurse Manager.

• Completes any County/State disease reporting forms timely and accurately.

• Completes data entry of patient clinical information for grant/contract requirements for County/State submission-- includes ARIES, Careware and other reporting systems.

• Communicates with patients between provider visits for condition updates, follow-through on the tests/referrals/procedures ordered by providers, responses to medications changes and adjustments, pre-visit preparations, and compliance/response to treatment plan changes.

• Reviews physician orders, laboratory requests and follow-up needs with patient.

• Maintains all Healthcare Center monitoring logs as directed by the Nurse Manager.

Clinical-Direct Care:

• Maintains standard precautions and all other precaution responses for infection control and according to OSHA requirements.

• Adheres to patient rooming practices as established by Healthcare Center operations policies and procedures.

• Collects samples such as urine, blood, sputum and stool specimens in the Healthcare Centers as ordered and instructs patients in home collection of urine, stool and sputum specimens, when appropriate.

• Completes treatments and procedures as ordered by the provider including EKGs, nebulizer treatments, wound dressings and other approved treatments.

• Performs phlebotomy as ordered and prepares specimens for transport to outside laboratory vendors. Checks patient’s insurance for identifying appropriate laboratory for sending patient specimens.

• Receives laboratory results over the telephone and transmits them to the licensed nurse and the patient's medical provider in a timely manner.

• Administers Purified Protein Derivative (PPD), as ordered.

• Administers medications and vaccines as ordered by the providers, and as allowed in individual states.

• Ensures appropriate processing of pre-operative orders including obtaining orders and faxing to specialist prior to day of surgery.

• Assists with referral processing, as cross-trained to include printing direct referrals, e.g., CXR, mammograms,etc.

• Reports changes in patient's physical status to the Nurse Manager, other nurses or medical provider.

• Assists nursing and medical staff with examinations and procedures as assigned.

• Communicates with responsible RN/Nurse Manager as trained in attending to telephone line to include returning calls, reporting laboratory results as allowed by provider.

• Receives and processes medication refill requests; verifies medication request with healthcare provider and obtains appropriate documentation.


Assignment Scope:

• Adjusts daily assignment and responsibilities as directed by the Nurse Manager.

• Provides coverage for evening clinic hours when requested.

• Provides coverage for satellite clinics when requested.

• Travels day-to-day to various Healthcare Centers, according to staffing needs and direction by Nursing Administration.


Professional Demeanor:

• Collaborates with other nursing personnel and participates as a team member.

• Communicates in a tactful and professional manner.

• Maintains appropriate garb without facial or body jewelry.

• Does not use personal cells phones or access internet for personal use during work hours. Cells phones are to be accessed only for emergencies.

• Arrives on time prepared to work at the beginning of the work shift. Adheres to 15 minute AM and PM break times and 60 minute lunch times—follows assignment to leaving and returning on time. Gives provider name of other nursing staff person covering while off the floor. Provides status report to covering staff member.


• Maintains supplies and cleanliness of examination rooms. Ensures cleanliness of patient rooms at end of each day.

• Cleans exam tables with appropriate disinfectant and replaces paper between patients.

• Maintains all Healthcare Center monitoring logs as directed by the Nurse Manager.

• Monitors patient care areas including laboratory and exam rooms for expired and damaged supplies and replaces as appropriate. Notifies Nurse Manager of supplies that require ordering and restocks to reasonable levels.


Participation in AHF Meetings/Committees
Attends staff and other meetings as directed.

Education and/or Experience
One year certificate from college or technical school; or three to six months related experience and/or training; or equivalent combination of education and experience.

Other Skills & Abilities/Qualifications
•Previous experience with medical front office a plus.
•Ability to work at various healthcare centers required.
•Computer literate.

Certificates, Licenses and Registrations
•Completion of an accredited Medical Assistant Program with certification.
•Current Cardio Pulmonary Resuscitation (CPR) License Required.
•Valid Driver License with proof of liability insurance.
•National Medical Assistant Certification
