Health Plans:

PHP is a Medicare Advantage Prescription Drug health plan specifically for Medicare beneficiaries who are living with HIV and reside in Duval, Broward and Miami-Dade Counties.
PHP is the first HIV-specific special needs health plan approved by the Centers for Medicare & Medicaid Services (CMS). This plan covers routine doctor and specialist office visits, emergency and urgent care, hospitalization, and more. It also includes a prescription drug benefit for no monthly premium.
PHC Florida
PHC Florida is a Medicaid managed care plan designed specifically for HIV-positive individuals who are eligible for Medicaid and live in Broward, Miami-Dade and Monroe Counties.  PHC Florida is a Managed Medical Assitance (MMA) plan offered through Florida’s statewide Medicaid Managed Care Program


The Transition of Care RN (RNTOC) blends the mission of AHF with strong nursing clinical skills to navigate our HIV/AIDS specialty health plan members through their healthcare process.  With a caring and knowledgeable focus, you will lead our care team to help facilitate a smooth modulation through the often difficult medical transitions of care.  You will complete hospital and post hospital assessments, ensure safe discharge planning, reconcile medications, and assure treatment plans are followed through to maximize clinical outcomes.

By utilizing core principles of nursing, you will meet our members where they are, identify strengths and opportunities for growth and assist them with realistic goal setting.   With your skills of collaboration and engagement, you will work with our interdisciplinary team to educate, reinforce, rehabilitate, and support our members to heal and grow to potentiate their health status and improve their quality of life.  Your assessments will directly contribute to the divisions review for medical necessity, under and over utilization and as a result improve our member’s health outcomes.  

The Transition of Care RN is often the first line of support to help facilitate the shift between levels of care. You will assess the member and re-establish the connection with their provider, provide support and education to the member by engaging our team of professionals and providing a plan of care to give the member vision and focus to heal.

The RNTOC can enhance the experience of other Managed Care employees by collaborating with the RN Care Team Managers to complete assessments and personalized care plans. You will improve relations with additional Managed Care and AHF departments by opening lines of communication between the member, health plan, and providers.
