We are currently looking for a Temporary DRS Case Manager at our SOuth Side Help Center Affiliate located in the city of Chicago, IL. This is a temp assignment with the possibility to turn into a full time position for the right candidate. This assignment is a minimum of 3 months. Please see responsibilities below.

YOUR CONTRIBUTION TO OUR SUCCESS!

Principal duties and Responsibilities:

 

  • Complete comprehensive psychosocial assessments, identitify needs, provide referrals, and assist clients with application process.
  • Conduct in home assessment with DRS customers to meet customer need for assistance with activities of daily living eligibility for program services. Conduct in office assessment for Medical customers.
  • Provide monthly home visit for DRS customers to ensure customer health and safety, supportive services needs are met.
  • Provide education, information and linkages to programs.
  • Assess customer eligibility for financial assistance programs and assist customer with applications as appropriate.
  • Review/revise service plans and perform follow-up to determine the quality of services provided to customer.
  • Provide and support customer in the follow up with medical care, medication adherence, risk reduction, nutrition, mental health and substance use.
  • Locate, identify and utilize community services and program.
  • Develop and maintain required records, report and continuing education requirements. Update client information in client centered data base within 3 business days of completion

Qualifications:

A bachelor degree in Sociology, Psychology, Social Work and/or equivalent experience or training in medical/nursing or related field preferred. Previous experience in HIV prevention services and knowledge of working with various populations, LGBT, and HIV+/PWA required. Driver’s License, insurance, good driving record and access to a reliable transportation. The candidate must pass the Standardize Case Management Competency Training provided by the AIDS Foundation of Chicago within a year, must possess excellent interpersonal, organizational, communication and analytical skills and computer literacy required with proficiency with Microsoft Office products preferred. Bilingual (English/Spanish) preferred.
