    Specific Responsibilities include:

    • Establishes a relationship with client to ensure their day-to-day and health specific needs are addressed
    • Identifies and works with client to eradicate barriers to HIV treatment and overall medical compliance
    • Ensures understanding of medical therapies and the connection to viral suppression
    • Provides ongoing treatment adherence coaching
    • Conduct intake and comprehensive assessments for development of Individual Service Plans
    • Advocate on behalf of clients to link them to services in the Greater Cleveland community
    • Assist clients in accessing government programs and benefits
    • Work with client to identify and establish a comprehensive support system
    • Collaborate with community agencies, services and medical personnel to serve clients
    • Follow case management guidelines established by the Ohio Department of Health and the National Association of Social Workers
    • Maintain client/service data information and complete reports and documentation
    • Participate in relevant meetings and activities both internal and external with community partners
    • Perform other duties as assigned

     Job Qualifications:

    • BA degree in relevant social service field
    • Knowledge of HIV issues and sensitivity to needs and concerns of people living with HIV/AIDS
    • Work experience in advocacy service, case management, or similar work with the public in a direct service setting
    • Experience working in a non-judgmental and supportive capacity with clients who experience psychiatric disabilities, chemical dependency, and/or life challenging diseases.
    • Flexibility to work in sites other than the Taskforce office and work in a cooperative/partnership program setting.
