    AIDS Healthcare Foundation is currently seeking a Part-Time Wellness Provider who will report to the Sr. Regional Program Manager and have daily reporting to the Medical Director for their outpatient clinics in Northern, California. This position works at our Oakland Wellness clinic on Mondays and Thursdays 2:00 pm-7:30 pm.

     

    A Mid Level Provider contributes to AHF's mission with a friendly and helpful manner to all patients and people seeking testing or medical assistance. The Mid Level Provider is responsible for projecting and integrating the mission and core values of the organization within and outside the Healthcare Center. Works with nursing staff and the front office to accommodate urgent care/walk-in patients. Ensuring all patients medical history is collected, formulates plan and prognosis based on patient's condition, coordinating patient scheduling, performing physical exams during medical visits, evaluates diagnostic test, prescribing necessary medication or other forms of treatment, and Refers patients to physician or specialist for consultation, encourage clients to continue to seek medical treatment from an AHF Healthcare Center and recommend AHF services to others.

     

    This position should provide outstanding service that enriches AHF’s reputation. Writes admitting orders as appropriate and gives to patient to take to the emergency room if being admitted. The Mid Level Provider can help ensure the patient is receiving the highest quality of care.

     

    Mid-Level Provider: 

     

    The Mid-Level Provider provides clinical services at the AHF Oakland Wellness Center. The hours for the position are Monday & Thursday from 2:00 PM - 7:00 PM. The successful applicant with be a Nurse Practitioner or Physician Assistant licensed to practice in the state of California. The Mid–Level Provider will provide clinical assessment for patients that are symptomatic for STIs and order appropriate treatment as needed. Experience working in similar settings, and/or with LGBT patients is highly desired, but not required. Primary responsibilities include:

     

    • Assesses and examines Wellness Centers patients; this includes treating the patients in a timely manner, acting as a medical resources, and providing guidance as needed.  Provides STD treatment.

     

    • Performs targeted physical examinations and preventive health measures within prescribed guidelines and according to policy and procedure manual.

     

    • Orders, interprets and evaluates diagnostic tests to identify and assess patient's clinical problems and health care needs.

     

    • Utilizes electronic medical records system to document patient care.

     

    • Prescribes medication for treatment of STD’s and Post Exposure Prophylaxis.
