Founded in 1989, we began as advocates with a message to be heard: Persons Living with HIV and AIDS who had few resources and demanded to be treated with dignity. Our name comes from the anonymous, well-known 19th century poem about children born on each day of the week. ‘Thursday’s Child as far to go.’ The name spoke of hope and promise during times when there was little of either. We began as advocates and started the region’s first independent HIV housing program. Today, an HIV diagnosis is a manageable chronic condition. HIV testing is the first step all should take: knowing one’s HIV status can make the difference between life and death. Those who know their status is positive can be linked to care and live a long and prosperous life; not knowing can lead to serious illness, even death. Today, Thursday’s Child keeps its promise of going farther; we now offer various programs to support better health outcomes, provide for humane and essential needs, and continues its work to end the HIV/AIDS crisis.

Broward House, Inc. is a not-for-profit community-based organization serving Broward County with a focus on HIV care and Prevention, housing, and behavioral health. Broward believes in the dignity and value of every person who walks through the door. You then may want to be a part of our Transitional Housing Program in supporting those living with HIV impacted by co-occuring mental health and addiction.
In addition to other responsibilities, this position will have the opportunity to:
- Complete an integrated assessment of individual client’s strengths, needs, abilities and preferences
- Develop relationships for transitional housing placements
- Develops an integrated and collaborative individualized care plan with a transition component.
- Provides the appropriate level of individual and group therapeutic intervention to support the stabilization and progression of clients

- Provides linkage and navigation for health care, supports services and housing stability based on individualized needs
- Develops and prepares groups to meet client needs, including transition and after care.
- Provides supports to other disciplines in the residential program including but not limited to nursing, assessment, milieu and Behavioral Health
- Meets with internal and external interdisciplinary treatment team members to coordinate client services and documents outcomes of meetings, including hospitals, health department and medical providers.
- Submits all documentation in an accurate and timely manner, including electronic records Achieves and maintains program established productivity levels.
- Engages clients in a strength-based supportive manner providing re-directions and guidance as necessary.
- Other duties as assigned.

In addition to other duties, this position is responsible for:
- Engages, counsels, manages, motivates and effectively works with high-risk, vulnerable patient population, including chemically dependent persons.
- Engages, counsels, manages, and effectively works with persons in crisis.
- Engages the community to locate individuals requiring health care that are not currently accessing or adhering to services
- Completes assessment an assessment of client’s strengths, needs, abilities and preferences.
- Develops an individual plan of care and/or transition plan based on identified client’s needs.
- Provides direct support to clients in implementing activities to meet case plan goals and objectives
- Arranges for services through linkage, referral and navigation services
- Provide supports to initiate and follow through with appointment and requirement for health care and ancillary support services, including attending appointments
- Provides HIV, STD, Medical Adherence and Risk Reduction Education to individuals
- Guides clients in apply and accessing eligible benefits
- Prepare and facilitate groups including life skills, social rehab and psycho-educational
- Supports clients in communication with external and internal providers
- Meets with internal and external interdisciplinary treatment team members to coordinate client services and documents outcomes of meetings, including hospitals, health department and medical providers
- Submits all documentation in an accurate and timely manner. Achieves and maintains program established productivity levels.
